REFERRAL AND ASSESSMENT
- No formal referral is required.
- Where services from other agencies are needed a
referral will be made to the appropriate agency.
- An assessment of eligibility will be made according to
State/Commonwealth guidelines.
- The service is for frail, elderly or disabled migrants who are isolated and lacking in
social supports.
- Clients must be from a non-English speaking background.
Multicultural Planned Activity Groups offer an
opportunity for elderly migrants to meet regularly for leisure activities, social interaction, gentle
exercise and to provide carers with a
break. Lunch and transport are provided at a very reasonable cost of $6.00.
Groups meet three times a week :
- Tuesdays - 9.30am to 2.30pm
- Wednesdays - 9.30am to 12.45pm
- Thursdays - 9.30am to 2.30pm

The aims of the Multicultural Planned Activity Groups are to;
- promote independence of group members,
- provide socialisation and support for isolated older migrants, and
- provide respite for carers.
Activities include:
- Outings
- Craft
- Social interaction
- Gentle exercises
The Multicultural Home Visitiation Service
provides volunteer visiting for frail, elderly or disabled and isolated migrants.
Help may be given with shopping and appointments.
The service is provided with the assistance of volunteers and is
generally free of cost to the client. However reimbursement of transporting or out-of-pocket costs
can be arranged between volunteer and client.
CONFIDENTIALITY
Client confidentiality across all services is highly stressed and is respected.
VOLUNTEERS
The Multicultural Home and Community Care services welcome the services of volunteers both for the
Visitation Program and for the Planned Activity Groups.

All volunteers used by the Multicultural Home and Community Care Services have access to in-service
training and formal support from the Geelong Migrant Resource Centre. Prospective volunteers are
screened and subject to police checks.
